The short version
- We collect what you type into our request form and basic technical information about your visit.
- We share your request with exactly one licensed insurance agency, the one named on the form before you submit. We do not sell your information to anyone else and we do not share it with other agencies, lead buyers or data brokers.
- Under some state laws, giving your request to that agency in exchange for a fee counts as a "sale" or "sharing" of personal information even though it goes to one named company you chose. You can opt out at any time on our Do Not Sell or Share page.
- We never ask about your health, and we never pass any form contents to advertising platforms.
Who we are
Affordable Healthcare Center ("we") is a marketing company. Contact: hello@affordablehealthcarecenter.com, 1-866-255-6637, [Mailing address — Peter to confirm]. We are not an insurance agency and we are not a covered entity under HIPAA; the agency that contacts you has its own privacy practices.
What we collect
- You give us: name, phone, email, ZIP code, number of adults and children in your household, an income range, your current coverage type, preferred language, and your consent choices.
- Automatically: IP address, browser and device type, the page you were on, the referring site, campaign codes in the link you clicked, and a consent certificate from a third-party verification service (TrustedForm by ActiveProspect) that records how the form was filled out. This is how we prove, years later, exactly what you saw and agreed to.
- We do not collect: health conditions, medications, pregnancy, tobacco use, Social Security numbers, or payment information.
How we use it
- To deliver your request to the one licensed agency named on the form.
- To send you a confirmation of who received it and, later, one message asking how it went.
- To keep the record of your consent that the law requires us to keep (up to 5 years for telephone consent records; the agency keeps its own records for up to 10 years under Marketplace rules).
- To honor opt-outs and keep a suppression list.
- To measure this website in aggregate (page views, form starts and completions), without form contents.
Who receives it
- The one named agency. Its legal name, license and office appear on the form before you submit and inside your consent text. It uses your information to contact you about health insurance as your consent describes, and is bound by contract not to resell it.
- Service providers that work only for us: website hosting and database (Cloudflare), consent verification (ActiveProspect), our customer-relationship software for the confirmation message, and call tracking if you call us. They may not use your information for their own purposes.
- Legal. When required by law, subpoena or to protect rights and safety.
Consumer health data privacy notice
Some state laws (including Washington's My Health My Data Act and Nevada's SB 370) treat information about seeking health coverage as consumer health data. We do not collect health conditions, and we do not currently take requests from residents of Washington or Nevada. For anyone, the categories we collect are listed above, the purpose is to introduce you to one licensed agency, and the recipient is that agency and our service providers. You may access, delete or withdraw consent at any time through the privacy requests page. We do not use geofencing around health facilities.
Advertising and cookies
We use analytics and advertising tags (Google Analytics, Meta Pixel) that record page views and whether a form was started or completed. They do not receive your name, contact details, household information, income or any answer you give. We do not build remarketing audiences from health insurance pages. Your browser's Global Privacy Control signal is honored as an opt-out of sale or sharing.
Your rights
Depending on your state, you may have the right to know what we hold about you, to correct it, to delete it, to opt out of sale or sharing and of targeted advertising, and not to be discriminated against for exercising these rights. Submit a request on the Do Not Sell or Share page, by email, or by phone. We verify requests by matching the email or phone on file and respond within 45 days. An authorized agent may submit on your behalf with written permission.
Texting
If you consent to texts, reply STOP to any message to stop and HELP for help. Message frequency varies; message and data rates may apply. We do not share text opt-in consent with any third party.
Children
This site is for adults. We do not knowingly collect information from anyone under 18. Household counts of children are a number only.
Changes
We will post changes here with a new date. Material changes to how we share information will be announced on this page before they take effect.
